Release of Conduct Information Waiver (FERPA Waiver)
Student conduct information is considered confidential. There are some circumstances under which a student may wish to share their conduct information with another party, for example: being accompanied by an adviser to a hearing, or permitting a parent to review conduct information. Students who wish to make such a request must fill out the Release of Information Waiver, which can be found here for currently enrolled students and here for formerly enrolled students. This form must be submitted by the student themselves.
Office of Student Conduct Dean’s Form
Beginning July 1, 2021 there will be a $10 charge billed to your Bursar for each requested Dean's Form. (Ex: You request five institutions are sent a dean's form, the charge is $50). If you require more than five Dean’s Forms, you will need to submit a second request. All subsequent requests will follow the same fee schedule. Fee waivers are available if you have a financial need.
Some transfer or graduate school applications and employers require verification of a student’s disciplinary record by the university; this verification is called a Dean’s Form at Indiana University. If you have been asked to provide such information to an external institution or agency, please complete this form. This form must be submitted by the student themselves and it requires authenication.
If you are no longer enrolled at IU or unable to authenicate with IU systems, please contact UITS at 812-855-6789 for help retrieving your username.

Fee Waiver Request
There are some Division services, like seminars or referrals, that may be eligible for a fee waiver under certain circumstances. For more information and to submit a request, go here.
We believe that student organizations are capable of making good decisions and engaging in a Culture of Care for the health and safety of others.
